Casing was lost since motor cato blew off the retainer and bottom centering ring. Inside of tube got a miner burns. Propellant chunks burned inside. One witness seen them when it came apart. Fairly certain there was a failure near the forward end of the motor. Boxes propellant came in were thrown away before launch, so I can't verify the date code.
