Defects in either the liner, the nozzle or both caused high pressure gases to leak into the case and blow out the aft retaining ring. The motor/casing (minus the aft retaining ring (which blew off) & the nozzle flew through the rocket, pushing out the NC & damaging the distal sidewall of the BT.
