I had a G150 in a scratch built athena/thor 2" with a 13 second delay thay was not drilled as it was just there as a backup for dual deployment. The rocket was slow off the pad then about 30-50ft in the air it shot off the payload section and flame came out the front of the rocket and it fell back down. When inspecting the motor I found a hole in front of where the delay grain was about the size of the charge well which is why the flame shot out the front. Also the propellant was only half burnt, think it was extinguished when it blew through.
