Case and aft closure damaged, unburned propellant grains and intact delay grain ejected.

Comments
Using GoPro video at 240 frames/second, failure occurred 0.013 seconds after first indication of ignition. Used manufacturer supplied igniter. The igniter wire is still seen in the nozzle after failure. I believe the igniter is plugging the nozzle enough to cause overpressure.
